You're probably just thinking of the description for the third print issue.

Not necessarily. It's possible that Ermac gets a chapter in which he works against Mileena. Kotal infers from this that they're on the same side, but in fact Ermac is trying to restore Shao Kahn.
 
Possible, but it sounds more to me like something any allies would say. As a hypothetical example, Johnny to Raiden "I thought we were the good guys?" Doesn't mean that Raiden betrays Johnny in the story.

I've played every MK title starting with Deception, and I had no idea who he was until I saw people talking about him on this forum.
