Outside of classic costumes, which cannot be acquired normally, only Mileena has a 3rd costume.

To address the topic: I personally like Kung Lao's default costume. Sektor and Cyrax have nice alternates, with NRS even going as far as re-animating their moves. Unfortunately, most alts don't add much to the original design. :(

I like liu Kangs shaolin alt and Ermacs alt.

Since we are talking about Alt costume I want to point something out. I hate to sound like a Perv but I just unlocked Sonyas alt so today so when I went to check it out in the Nekro I toggled the damage and found out that sonya has a tanning line on her a** in the shape of her underwear.

Franky I like most of these costumes in this game hahaha. I am currently working on sculpts of the MK9 Scorpion and Noob Saibot... can't wait to finish these costumes!
 
All classic costumes. Noob's primary, Kung Lao's primary, Reptile's primary and alternate, Johnny Cage's alternate, Nightwolf's alternate.

Kitana's MK3 costume (the dark blue makes the outfit look much nicer than Mileena and Jade's MK3 costumes IMO)

Honorable mention to Sheeva, Raiden, Cyrax and Jax's secondary costumes too

I'm surprised no one mentioned Smoke's default. Man, what an overhaul that completely fits the character! His long, flowing hair is perfect for the flowing mist appearance of actual smoke, and the outfit itself is dark and compliments this motif as well. Of course the ladies have some sexy outfits (for the cloth that's there, and I'm certainly not complaining!), but Smoke's outfit blew me away with its intelligent and creative update for design.
 
The alt outfits for Sheeva, Kano and Smoke all kick butt.

@iluspook: I think Smoke's default is one of the worst, personally. The hair is horrible for a ninja and the crappy soft-body physics makes it even worse.
