I guess when they say "one female fighter is depicted partially nude, covered by a costume of thin cloth strips," they're referring to Mileena's flesh pit costume. It is unlikely that they NRS hid it from the ESRB, because you can get in big trouble for hiding content from the ESRB. I think that Mileena uses her hair to cover her boob's, and wears a loincloth. But that's just my speculation.
