Iv went and looked a whole lot and found nothing, iv tried every possible combination of ways to win the fight and nothing. Im fairly sure its mileena it refers to.
 
Side note I heard toasty bonus deals with a random super boost poping up instead of Dan forden and I'm still thinking jades in goros stage

some time ago some guy (in demo version) claim that when toasty appears he pressed R2. he tried to reproduce it without luck, so i suggest him to press Down+R2, i don't know if it's work or not.
that guy said the words "toasty buster" appears in the screen..

and about jade, i've read it in some post (can't find it again :P) it was against shao kahn, so the clue is about jade and ladder tower

Ahhhhh, I got the smoke fight!!!!

But I got it a little differently than what's been said..... Or maybe I didn't see him...... Anyways I got the forest on my first try around, but I didn't see Smoke appear so everytime before I won, I just pause and selected the character select and went back to the beginning and I would reload in the forest. This was about the 3rd time, and right as I was about to beat Cyrax (I don't think that's important), I kept hitting down and select like crazy, but when I beat him (flawless mind you) he exploded the background went black!!!!!

Gonna see if I can hold my own

Seems like there's more then one way to skin a hidden fight. :D
 
Lol I kept hitting down and select like crazy the whole time as I wasn't sure if Smoke appeared in just the middle of the screen, but I guess I must have hit when he popped out for a split second, pics in a sec
