How is this achieved?

You know, when you see someones face pixelated out. PS>Filters>Effects>Mosaic... then what? I know it's something like that.

When you make a new layer, how do you paste onto that layer? Is it just copy/paste or cut/paste whatever?

Well, i would think you could just blur it, or use a mosaic tiles filter...i've never tried frankly, but I'm sure I could manage something messing around with filters.
