I don't think they've ever really explained this. I assume he has it because his eye was gouged out at some point, but beyond that speculation, I have no idea.

My guess would be that Boon doesn't even know.

I like in the old movie when Shang Tsung threatens Kano with the knife and his eye twitches, I think that could easily be used to explain something about it.

Hmm...well, we know it made from the same kind of design as Hsu Hao's bionic heart...maybe his original eye was gouged out...maybe he did it for the power? But it's like Deathcore said, it's doubtful even Boon knows (especially since Tobias always handled the character design and story, afaik).
