I may be wrong but I'm going to say that's fake. I owned the original snes cartridge and it looks nothing like that and I don't recall ever seeing it in ANY video game store. And I'm not talking about major retailers. I'm referring to mom and pop game stores that usually have used game cartridges out on display. And I've been to many of them throughout my time as a gamer, which is about 22 years, and not once have I seen this.

no fake dude i have one , and beside was few cartridges relased of this ,, was the first version but very limited or hard to find i opened mine and is original i own this from 90 th yearsIMG-20160112-WA0004.jpg

I've never seen this, and would have concluded it as a fake as well.

Does anyone claiming it's an alternate rare cover have any sources to back up this claim? Just because a few people claim to have one doesn't mean it isn't something someone made a few copies of and sold on eBay.
 
That still doesn't mean someone didn't make a few with these alternate stickers and re-seal them for sale. It happens all the time in the retro-gaming community.
 
The only other info on this alternate cart is over at MKOnline, in which someone claims to have gotten it in 1995. Idk why these aren't spoken about more often. I've never seen one in person, EVER. What country are you in?
 
im from costarica ,, and here too was rare or is rare to find just few people i own this game from 1992 i can't remember if you open the cartridge you can see is original and no reimpresed the stiker here in my country we was coment about this cartridge and there is some few people who, own or saw this version too

by the way do you remenber the first xbox classic controllers,, bigger and then modifiqued ,, or famicon controller with square botons and then circular ,, maybe was the same with this cartridges ,,
